Undiagnosed hypertension is a significant issue in Cheshire and Merseyside, with many people unaware of the risks. To tackle this, we developed a new brand, digital tool, and social media campaign designed to encourage conversations about blood pressure and overall wellbeing within the community.
What We Did
To support local pharmacies and health trainers in offering blood pressure checks, we created the Happy Hearts web app. This simple yet effective digital tool facilitates a 2-3 minute conversation between health professionals and patients, encouraging them to discuss their lifestyle, interests, and heart health. This approach aims to increase patient willingness to undergo a blood pressure check.
The project was based on qualitative research that explored ways to motivate people to adopt healthy lifestyle changes, focusing on risk factors, self-care, and medication adherence. The insights gathered informed the development of the brand, identity, and user journey for the tool, ensuring it supported both patients and health professionals effectively.
The tool was developed in close collaboration with pharmacies, health trainers, and patient representatives to ensure it met the needs of all users. Feedback from these groups helped shape the app’s content, format, and tone of voice, creating a product that resonated with the target audience.

Alongside the app’s launch, we provided a training package for professionals, boosting their confidence in using the tool. Following the launch, we rolled out a targeted social media campaign using static and animated content to engage diverse audiences across the region.
